Non-esophageal eosinophilic gastrointestinal disorders are difficult to diagnose because symptoms overlap with functional dyspepsia, IBS, celiac disease, inflammatory bowel disease, infection, and medication effects. Endoscopic appearance can also be normal or nonspecific.
Multiple biopsies matter because eosinophilic inflammation can be patchy. Samples should include normal-appearing as well as abnormal-appearing duodenal mucosa, and pathologists need the specific anatomic site because normal eosinophil density varies throughout the gastrointestinal tract.
There is not one universally accepted adult diagnostic threshold. Yes. Non-EoE eosinophilic gastrointestinal disorders can relapse after improvement or during treatment reduction. Recurrence may involve symptoms, tissue inflammation, or both, so long-term follow-up is individualized.
Persistent disease may contribute to chronic pain, vomiting, diarrhea, weight loss, anemia, low protein, nutritional deficiencies, ulceration, and reduced quality of life. More extensive or deeper eosinophilic disease can rarely cause obstruction or other complications.
Response can be tracked through symptoms, weight and nutrition, blood abnormalities when relevant, and repeat endoscopy with biopsy in selected patients. Symptoms alone may not reliably show whether microscopic inflammation has resolved.
