Your Health Records and Consumer Rights in Texas
Texas law gives you the right to know how to get your health care records, how to contact the agencies that license our clinicians, and how to file a consumer complaint. This page explains all three.
1. How to Request Your Health Care Records
You can ask for a paper or electronic copy of your GastroDoxs medical and billing records.
- By phone: call 832-632-4070 and ask for Medical Records.
- In person: ask at the front desk of any GastroDoxs office (Cypress, Jersey Village, Katy or Brookshire).
- By mail: GastroDoxs PLLC, Attn: Medical Records, 10425 Huffmeister Road, Suite 280, Houston, TX 77065
- By email: info@gastrodoxs.com (please include only your name, date of birth and a phone number; we will contact you to verify your identity).
Please tell us your full name, date of birth, the records or dates of service you need, and whether you want paper or electronic copies. We will verify your identity before releasing records.
We will provide your records within 15 business days of your written request. Electronic copies are free; for paper copies we may charge only a reasonable, cost-based fee allowed by law. If you want your records sent to another person or provider, we will ask you to sign an authorization.
